Mandated Reporting Procedures

  1. Program Director/Responsible University Officials are Mandated Reporters who are required to report child abuse or neglect within 24 hours by calling:
      • 1-855-GACHILD (1-855-422-4453)
        Georgia Department of Human Services (DHS)
        Division of Family and Children Services (DFACS)
        Child Protective Center
      • 706-507-8911
        ´ó·¢ÌåÓý¿ª»§ University Police Emergencies
  2. The Program Director/Responsible University Official/Mandated Reporter should also notify the ´ó·¢ÌåÓý¿ª»§ Program Sponsor, by phone, email, or in person within 24 hours.

Incident Reporting Procedures

´ó·¢ÌåÓý¿ª»§ has multiple avenues to report all types of incidents or concerns. To report suspected child abuse or neglect and/or accident/safety incidents in programs utilizing ´ó·¢ÌåÓý¿ª»§ facilities and that involve unenrolled minors, the following procedures should be followed:


  • To report University-related incidents/accidents involving employees, students, visitors, and contractors and to submit an ´ó·¢ÌåÓý¿ª»§ Incident Report and Claim Form Accident Information – General Liability.

To report other types of incidents on ´ó·¢ÌåÓý¿ª»§ campuses the following additional avenues are available:


  • This is the website to report incidents related to: an alleged incident(s) of discrimination, harassment (including sexual misconduct), or retaliation.
  • Create Care ´ó·¢ÌåÓý¿ª»§ Reporting
    Create Care ´ó·¢ÌåÓý¿ª»§ is a network made up of all members of the ´ó·¢ÌåÓý¿ª»§ campus community, including you, that work together to promote a safe and successful environment. To share information about a person and observed behaviors and statements that generated the concern.

  • This is the website to report unethical behavior and policy violations related to: violence and assault; violations of the university policy and procedures; housing, auxiliary services (parking, transportation, bookstore, card services, culinary and hospitality, copy/print services, student health services, retail shopping, postal services), ADA/disability, student activities, university events, facilities, or any other related services; financial aid or registration, records and transcripts; and discrimination. You may choose to provide your contact information or to remain anonymous.
